Our Mission
The Health Experience Partnership Program aims to foster effective communication and collaboration between patients, families, and staff. We promote patient and family advocacy, meaningful involvement, and co-design of healthcare initiatives. Through empowerment and support, we enable patients and families to actively drive positive change in their healthcare services.
The Core Team
Our Health Experience Partnership Program is led by a dedicated core team comprised of individuals who have personal experiences as patients, family members, and caregivers—just like you. These Core Team members serve as a voice of our Health Experience Partners, ensuring that your perspectives and insights are heard and valued. The Core Team works collaboratively to foster meaningful partnerships and drive positive change in our healthcare services with support from and in collaboration with Quinte Health staff.
Our Health Experience Partnership Program recognizes the valuable insights and perspectives that patients, families and caregivers bring to the table. Co-Design, which is collaborating with patients, families and caregivers in the design of programs, services, policies and more, allows us to collaborate and learn from lived experiences in our health system to make our system better for everyone.
Involving patients in a co-design role and an advisory role within a healthcare setting represents two distinct approaches to patient, family and caregiver engagement.
Co-Designing:
Partners actively participate in designing, developing, and improving healthcare services from the beginning
Viewed as partners collaborating with healthcare providers and stakeholders
Equal contribution in shaping policies, programs, and practices
Ongoing involvement throughout the project lifecycle
Fosters a sense of ownership and accountability
Emphasizes patients’ needs and preferences
Advising:
Advisors provide input and feedback at a later stage of decision-making
Invited to provide input, feedback, and recommendations
Participation in committees, focus groups, or feedback sessions
Scope and duration of involvement may be more limited
Valuable role in informing and influencing healthcare decisions
Improved Health Outcomes:
Active patient involvement leads to better health outcomes and treatment adherence
Engaging patients in their treatment plans improves overall health and well-being
Enhanced Patient Satisfaction:
Empowered patients experience higher levels of satisfaction
Patient engagement results in a more positive healthcare experience
Better Care Planning:
Patient engagement ensures personalized care plans
Involving patients in care planning respects their individual needs and preferences
Increased Safety:
Patient partnership helps identify and prevent errors or adverse events
Patient insights contribute to enhanced safety measures
Improved Communication and Shared Decision-Making:
Patient engagement fosters open and effective communication
Shared decision-making empowers patients in determining their care options
