Diabetes Education Program
The Diabetes Education Program at Quinte Health works in collaboration with other provincial and regional organizations and agencies.
The model is a consultative service for professionals and individuals (clients/patients/families) that are living with diabetes. The diabetes team helps the individual to self-manage their diabetes.
The interprofessional team consists of certified diabetes nurse educators and certified diabetes dietitians.
The Paediatric team also has a pediatrician and social worker.
Referral to the Diabetes Program is by physician or self-referral from the client/patient/family by calling the diabetes program.
