Symptom Management/Palliative Care

The Palliative Care Nurse Consultant is part of the larger Quinte Health interprofessional team.

The Palliative Care model is a consultative service for professionals and individual clients/patients/families that experience issues related to pain, symptom management and psycho-social-emotional needs.

These services are provided to the adult inpatient populations. Support is also offered to individuals and their families transitioning from palliative to end-of-life care.

Referral to the nurse consultant is by all professionals and by self-referral from the client/patient/family.
