Transitional Outpatient Centre

The Transitional Outpatient Centre (TOC) is Quinte Health’s outpatient mental health clinic. Staffed by a team of Registered Nurses who work in collaboration with Quinte Health psychiatrists, TOC provides services for clients experiencing serious psychiatric/mental health issues. TOC is one aspect of Quinte Health’s mental health service continuum and is focused on supporting clients either following an admission to the psychiatric inpatient unit or psychiatric consultation.

Services provided by the Transitional Outpatient Centre are aimed at preventing admission and readmission to the inpatient unit, as well as reducing the length of stay in the unit by providing client-centred counselling and support at times of increased vulnerability. TOC provides psychoeducational programs in a didactic group or individual counselling format dealing with such subjects as: emotion regulation, distress tolerance, behavioural activation, cognitive restructuring, assertive communication, problem-solving, healthy coping, sleep hygiene, stress management and relapse prevention. These services are offered to clients with mood, anxiety and adjustment disorders.

TOC also offers programs for people with chronic mental illness such as schizophrenia and other psychotic or mixed mental illnesses. For these individuals, treatment is provided in a setting where psychiatric assessment, medication monitoring and administration, and psychosocial rehabilitation therapy are readily available. Case planning in conjunction with the client community care teams is also provided where appropriate.

Referrals must be consulted under the care of a Quinte Health psychiatrist.

The hours of operation are Monday to Friday 8:00 a.m. to 4:00 p.m. The Transitional Outpatient Centre is found in the Sills Wing at Belleville General Hospital.
