After three years of hard work from our Best Practice Guideline (BPG) Champions, the Registered Nurses’ Association of Ontario (RNAO) has designated Quinte Health Care a Best Practice Spotlight Organization (BPSO)!

“Achieving our BPSO designation at Quinte Health Care has been an incredible journey and we could not have done it without the support and engagement of our BPG Champions,” said Terry Holland, Professional Practice Coordinator and BPSO Lead for Quinte Health Care. “We are thrilled to have received this designation and are dedicated to continuing our commitment to excellence in the care that we deliver every day.”

The following is a little bit of information about the great work that has occurred for each of our seven Best Practice Guidelines:

Assessment and Management of Pressure Injuries for the Interprofessional Team (corporate)

  • Updated skin and wound care products
  • Revised patient education pamphlet and staff Wound Products resource
  • Changes to terminology and other documentation
  • Education and competency evaluation to all nursing staff (i.e. PI staging, prevention, management)
  • New eLearning module for Pressure Injury Staging in QHC eLearning Portal

Preventing Falls & Reducing Injury from Falls (corporate)

  • Documentation review
  • Falls Policies review and updates to standardize practices
  • New patient education pamphlets and posters
  • Significant improvements to falls risk assessment compliance and post-fall assessments

Assessment and Management of Pain (emergency department)

  • Improved compliance with pain screening at triage
  • New Lego pain assessment tool rolled out to emergency departments and plan for a different tool for “older persons” who are unable to self-report pain
  • Amazing engagement with patient experience partner in Quality Improvement work and new pain strategies
  • White Rose Program, new resources for opioid use
  • Improving timing of pain-reassessments following initial screening in triage

Assessment & Care of Adults at Risk for Suicidal Ideation and Behaviour (mental health and emergency department)

  • Columbia-suicide-severity rating scale implemented
  • Collaborative Care Planning and updates to documentation
  • New & revised assessments
  • Improvements to orientation & mentorship
  • Education and staff engagement

Person & Family-Centred Care (maternal/child)

  • More strategically engaging patients and families in the design of our care processes
  • Patient Advisors and new Family Experience Survey
  • Admission and discharge packages and other resources for patients/families/staff
  • Journal for parents to document their journey in the Special Care Nursery
  • Policy improvements – Visitors process

Preventing Violence, Harassment and Bullying against Health Workers (corporate)

  • Collaborations with Occupational Health, Safety and Security teams
  • Staff education across multi sites and units
  • Improvements to DASA documentation and new care planning
  • New eLearning module for the DASA
  • Improvements to communicating identified risk, including changes to the SBAR

Care Transitions (corporate)

  • Patient-Oriented Discharge Summaries
  • Staff education across multi sites and units
  • COPD discharge transitions program
  • Changes to interdepartmental hand-off report
  • Discharge transition phone calls underway
